School Based Health Center
Brookfield Public Schools and The CT Institute for Communities (CIFC Health) are working together to support students' and families' needs by providing mental health services within the high school.
What is the School Based Health Center?
As a student at Brookfield High School (BHS), your child can take advantage of mental health services offered during school hours, through the School Based Health Center (SBHC). A Licensed Clinical Social Worker will be on site to provide care like what you would find in a community-based mental health provider’s office.
Parents/guardians don’t have to miss time from work to take their child to an outside provider for many mental health services, since they are provided at the SBHC.

How to enroll?
Enrollment in the SBHC is offered and accessible to every student attending Brookfield High School, regardless of their ability to pay. If your child is not covered under health insurance, please call CIFC Health's Financial & Insurance Assistance Department at 203-448-2668.

OPTIONAL: Families have the option to permit the SBHC therapist and BHS school staff to communicate with each other. Please complete this form if you wish to allow for communication between BHS school staff and the SBHC provider. If chosen, the optional form should be returned directly to Chelsea Porter, LPC, Licensed Clinical Social Worker at fax or email (below).
