In order to properly register at Blackham School, we would need the following documentation

prior to your arrival:

• Two proofs of address (lease, mortgage payment and utility bill - not cell phone)
• Birth Certificate
• ID
• School Documentation (IEP, 504 Plan or Exit Papers)
• Medical Records
