If your child is currently attending Lake Forest Middle School as an eighth grader, your child will automatically be enrolled to attend Bradley Central High School. However, there are required forms that must be completed in order to comply with local and federal policies. Please complete the three forms below to register your child for their freshman year.
1 - [Online] Bradley County Schools Registration Form
2 - [Online] TN Migrant Education Program: Occupational Survey
3 - [Printable] Home Language Survey (Must be completed once.)
English - Home Language Survey
Español - Encuesta Sobre el Idioma del Hogar
Русский - Опрос по домашнему языку
If your child attended school in a different school district and is transferring to Bradley Central High School, parents/guardians will need to complete these forms:
1 - [Online] Bradley County Schools Registration Form
2 - [Online] TN Migrant Education Program: Occupational Survey
3 - [Printable] Home Language Survey (Must be completed once.)

4 - Must submit a copy of these records to BCHS: students birth certificate, social security number, Tennessee Immunization form, grades, discipline, and attendance from the previous school.
Acceptance of out-of-zone enrollment requests is based on several factors. Once your child's records are reviewed, expect to receive a letter/phone call stating acceptance status to BCHS. Parents/guardians of students who reside within the Bradley County School district and who are zoned to attend Walker Valley High School based on their permanent address, will need to complete these forms.
