RMS Athletic Registration 2023 - 2024
2023-2024 ATHLETICS REGISTRATION
The following four steps must be completed in order to be eligible to participate in RMS Athletics. (This is a separate process from BPS power school registration)

REVIEW REQUIRED REGISTRATION DOCUMENTS
When completing the online registration form, parents/guardians and student athletes will be required to verify that they have read and agreed to each of the following documents. These documents may be printed out for reference only. DO NOT turn in these forms or documents.

2-IHSA SPORTS MEDICINE ACKNOWLEDGEMENT AND CONSENT FORM -LINK
Parents/guardians and student athletes will electronically acknowledge this form on the registration document DO NOT PRINT THIS FORM OUT
Includes: concussion information, performance enhancing drug information and asthma medication consent.
If your son/daughter requires asthma medication (self-administered), the following must be done:
Print out the last page of the document
Complete the form and turn it in to the Rotolo Main Office

Parent/guardian must complete and submit the above form in order to register your son/daughter for athletics at Rotolo Middle School. Please register each child individually. Only register once, unless you have more than one child participating in athletics at RMS. The above form includes emergency contact info, parent consent and verification that parent/guardian and your student have reviewed all required registration documents.
3. PROVIDE RMS WITH A CURRENT PHYSICAL
6th grade Students:
6th graders are not required to have a separate sports physical in order to participate for RMS athletics. The regular school physical that is required to enroll in middle school satisfies this requirement. This is the only document that 6th grade students do not need to turn in.
7th and 8th grade students:
7th and 8th grade students, must have a current physical on file in order to participate. A physical is good for 13 months, any physical older than 395 days is invalid and will not be accepted.

Please drop off the completed sports physicals to the RMS main office (Monday- Friday, 8 am - 3 pm) starting July 17th. There will also be a drop off station during August 9th Registration.
Fall Athletes must turn in prior to the start of the season which begins the week of August 24th.
4. PAY THE PARTICIPATION FEE
Prior to the start of your athletic season, all participation fees need to be paid in one of two ways:
online through Power School beginning early August
checks made out to Rotolo Middle School ($150.00) and dropped off to the RMS main office.
Please note that some sports may charge additional dues to cover equipment and/or equipment fees.
Please note there are no refunds for RMS sports once competitions start, regardless of injury, eligibility, or an athlete deciding not to continue playing the sport.
Please note that solely paying for athletic fees does not fully register your son/daughter for athletics at RMS. The above three steps must be completed in order to fully register your son/daughter for athletics at RMS.
Parent/guardian should not pre-pay for sports their son/daughter may choose later in the school year.
