Forms
All medical forms must be submitted propr to the start of school in August. Completed forms can dropped off at the Education Administration Center on Evergreen Road care of Renee Ruiz over the summer. Completed forms can also be emailed to Renee Ruiz at rruiz@birmingham.k12.mi.us.
The following forms are required to be completed before the start of EACH school year for any qualifying student:
Permission to Administer Medication
Asthma Action Plan
Seizure Action Plan
Diabetes Medical Plan
Food Allergy Plan
