To refer your student or child to the Jackson-Mann Student Support Team (SST), please fill out this referral form with as much information as you have.

Within one week of your completion of the SST referral form, you will be contacted by a member of the team to discuss your concerns and schedule a meeting with the SST.

What you can expect:

• An SST member will be assigned to you to help gather data and information

• That SST member will help you fill out the pre-SST meeting form

• You will be invited to attend a 30-minute SST meeting

• During that meeting, the SST will create an intervention plan for your student

• An SST member will help you track progress of the intervention

• You will be invited to another meeting to discuss intervention results
