Transfer and Appeal Form received by August 31st - Eligible for a Fall Evaluation
Transfer and Appeal Form received between September 1st and October 31st - Eligible for a General Evaluation to determine eligibility for the following school year
Transfer and Appeal Form received after October 31st - Consideration will be deferred to the following school year
Transfer Student Form - GT Eligibility Request: LINK to complete the form if your child was formally identified as Gifted and Talented in another state or in a private Kentucky school. Permission to Evaluate Form - Please note, we will also need this form LINKED HERE completed, signed, and uploaded.
Parent/Guardian Appeal Form - LINK to complete the form if you are requesting an appeal for an additional review of your child's eligibility and/or request an evaluation for an additional identification area. OPTING OUT of Gifted and Talented Services
Parents my choose to decline services in one or more areas in which their child is identified for Gifted and Talented Services. When services are declined, they are declined for the current school year only.
To opt out of Advanced or Accelerated Coursework, please complete this form and return it to the student's school G/T Lead.
To opt out of any other G/T services, please send an email to the student's school G/T Lead. Sample text for email:
