The forms below are required to be updated annually:
Idaho School Immunization Requirements
All incoming 7th grade students are required by the state of Idaho to have two additional immunizations: The Tdap and the Meningococcal vaccines. If you choose not to immunize your child, you MUST have an exemption form on file for these vaccines.
New to district/state students: Please review your student’s immunization status with your physician to ensure that your student meets the Idaho Immunization requirements. Many surrounding states do not require the same immunizations as Idaho for school attendance.
Athletic /Physical Education Forms
All incoming 7th grade students are required to have a school physical completed AFTER MAY 1st, prior to entering 7th grade. If your student chooses to participate in school sports, they will need to have another school physical again in 9th grade and their 11th grade year.
The physical exam forms must be loaded onto your studen'ts Aktivate account. Please do not bring a hard copy to the Health Office.
