Health History

Please update your students Health History each year before returning to school. This form gives health services permission to administer PRN medication as needed per parents request.

Nurse Curriculum

  • In Kindergarten, 1 st and 2 nd grades students will learn about types of germs and how they are spread. In addition the students will learn healthy behaviors that will reduce their chance of becoming sick, and how to protect themselves.

  • In 3 rd grade students will continue to learn about germs and their role in causing disease. They will learn about the immune system and how it works to keep us healthy. We will also discuss how to prevent injuries and practice daily healthy habits.

  • In 4 th grade we will focus on the prevention and control of disease. We will talk about the prevention of common childhood injuries, illnesses, and identify a variety of healthy practices and behaviors that maintain or improve health. We will discuss communicable diseases and explore the role of components of blood relating to the immune system. We will learn how vaccines work and the outcome of various options when making health related decisions.

  • In 5 th grade students will have a lesson that focuses on growth and development. They will learn about the physical characteristics and emotions that accompany adolescence and puberty. They will also learn about appropriate hygiene and health needs. For this lesson we will separate students by gender. The students may also have this as an optional lesson in 6 th grade.

  • In 5 th and 6 th grades, students will continue to learn about communicable and noncommunicable illnesses and how to make good choices in regard to their own health. One specific lesson will address Bloodborne pathogens (HIV, Hepatitis B and C) and how students can protect themselves by not touching other people’s blood or contaminated needles. In addition to this, 6 th grade students will learn about sexually transmitted diseases. Prevention of these diseases will focus on abstinence and saying no to illegal drug use.
