Below you will find the documents for the marching band season.
ALL students need to fill out all parts listed below of the physical form to participate in marching band.
This includes:
State Athletic Physical Form SIGNED BY Physician (4 page document you can print below) - due no later than 8/7
State Concussion Policy (online at Marching Band Google Classroom) - due no later than 8/7
Cardiac Pamphlet Sign Off (online at Marching Band Google Classroom) - due no later than 8/7
State Steroid Policy Consent (online at Marching Band Google Classroom) - due no later than 8/7
Emergency Info (online at Marching Band Google Classroom) - due no later than 8/7
Opioid Fact Sheet (online at Marching Band Google Classroom) - due no later than 8/7
Health History Form (online at Marching Band Google Classroom) - due no later than 8/7 if your exam was 90 days prior to 8/14/23
Medications:
If your student needs to bring medications (either Prescription or Over the counter) they need to fill out the Prescription Health Consent.
If your student needs to carry an inhaler they need to fill out the Asthma Inhaler Consent Packet.
If your student needs to carry an Epipen they need to fill out the Epipen/Food Allergy Consent Packet.
**The prescription, epipen, & inhaler documents will only be sent to you if you state you need them when registering.
ALL students need to fill out all the documents in the MB Commitment packet (this is a hard copy packet that will be sent to student upon contacting Mrs. Kerr).
This includes:
Band Camp Permission Slip
Marching Band Season (Games, Comps, etc.) Permission Slip
Marching Band Festival Letter
School Computing/Publicity Consent
Registration Form/Commitment Contract
