Medicare Appointment

Are you 65 or turning 65 in the next few months? Or are you disabled? If you answered yes to either of those To book your personalized Medicare appointment please call the office at (419) 767-9001 opt. 1 or email Bridget@bjosephinc.com or stop in the office during regular business hours.

What you will need to bring with you to your appointment:

- A list of all your medications (pill bottles work the best)

- A list of all your current doctors such as family doctor, dentist, eye, specialist, etc.

- Your Medicare card if you do not have it I will not be able to start a policy for you - you may also call 800-medicare to get your member ID number and Part A&B effective dates 

- Your Medicaid card on Medicaid 

- A form of payment ( not all plans have a fee )
