VOE Form

The secretary at the front desk at Crossroads High School takes care of Verification of Enrollment (VOE) forms for Crossroads High School Students.   Students need to let her know they need a VOE form.  She will process the paperwork and return it to them.   Please give 24 hours' notice when your student needs a VOE.

VOE forms are valid for 30 days from the date given and students will need one every time they go to the DMV.
