Determine if You Need a Re-Issuance
Visit the ID Production Office as WALK-INS
Lost ID
Damaged ID
Worn-out or unreadable RFID card
ERROR (Information, Prints)
Changes or Updating of Information.
Step 1: Secure a Notarized Affidavit of Loss
Obtain a notarized Affidavit of Loss. You may visit the Public Attorney's Office (PAO), where notarization is generally provided free of charge (subject to eligibility and PAO policies).
Step 2: Pay the Re-Issuance Fee
Proceed to the BiPSU Cashier's Office and pay the required School ID Re-Issuance Fee.
Step 3: Complete the Online Application Form
Fill out the official School ID Re-Issuance Application Form using the provided link.

Upload the Required Documents

Step 4: Visit the ID Production Services Office
Proceed to the ID Production Services Office as Walk-in.
Step 5: Present the Original Documents
Submit the following for verification:

Original Notarized Affidavit of Loss
Step 6: Accomplish the Printed Transaction Form
Fill out and sign the printed transaction form provided by the ID Production Services Office.
Step 7: Wait for ID Processing and Printing
Once your documents have been verified, your School ID will be processed and printed. Please wait for further instructions regarding the release of your new ID.

Step 1: Pay the Re-Issuance Fee
Proceed to the BiPSU Cashier's Office and pay the required School ID Re-Issuance Fee.
Step 2: Complete the Online Application Form
Fill out the official School ID Re-Issuance Application Form and upload a clear copy of your Official Receipt (OR). You must also acknowledge and agree to surrender your damaged School ID during your office visit.
Step 3: Visit the ID Production Services Office
Proceed to the ID Production Services Office and:
Present your Original Official Receipt (OR).
Surrender your damaged School ID.
Accomplish and sign the printed transaction form provided by the office.
Step 4: Wait for ID Processing and Printing
After your documents and damaged ID have been verified, your new School ID will be processed and printed. Please wait for further instructions regarding the release of your replacement ID.

Step 1: Pay the Re-Issuance Fee
Proceed to the BiPSU Cashier's Office and pay the required School ID Re-Issuance Fee.
Step 2: Complete the Online Application Form
Fill out the Application Form and upload a clear copy of your Official Receipt (OR). You must also agree to surrender your current School ID during your office visit.
Step 3: Visit the ID Production Services Office
Proceed to the ID Production Services Office and:
Present your Original Official Receipt (OR).
Surrender your current School ID.
Step 4: Accomplish the Printed Transaction Form
Complete and sign the printed transaction form. If updating your ID picture or digital signature, a new photo and/or signature will be captured during your visit.
Step 5: Wait for ID Processing and Printing
Once your request has been verified, your updated School ID will be processed and printed. Please wait for further instructions regarding the release of your new ID.

Important Notice:
The information printed on your School ID is based solely on the details you submitted through the online application form. Before printing, you were given an opportunity to review and confirm your information on-screen. If incorrect information was approved and printed, a School ID Re-Issuance is required, and the corresponding re-issuance fee must be paid.

Surrender your current School ID.
Accomplish and sign the printed transaction form.
After your documents have been verified, your corrected School ID will be processed and printed. Please wait for further instructions regarding the release of your new ID.
