Click on the link above to fill out the Medical Release/Waiver form

Scroll down to the Club Selection section and choose HEARTLAND VOLLEYBALL CLUB, Milaca, MN from the drop-down selection:

Scroll down to the Contact information section and complete all required fields

Wally Brown is the Club Director and the email address is hvbced@gmail.com

Scroll to the participant information section and fill in all required fields for the player.

In the required Team Name field use Big Lake XX where XX is your players age

Continue completing each waiver section:

Then Click Submit
