Welcome to The Center’s health services page. The Center employs two full time nurses to assist with the unique health conditions of our students. Below you can find links to blank physician order forms (for medication administrations, tube feedings, ect.), blank seizure action plans (required for all students diagnosed with seizures to attend The Center), and blank copies of the Illinois State Board of Education required examinations. Most Common Reasons Children Stay Home From School
Fever of 100 degrees or more-Please keep your student home for 24 hours after fever is gone without using fever reducing medication such as Tylenol or Motrin.
Vomiting and Diarrhea- Keep your student home for 24 hours after the last episode.
Sore Throat-If your student has tested positive for strep, your child must stay home for 24 hours after antibiotics have been started. Additionally, please keep your student home for 24 hours after fever is gone without using fever reducing medication such as Tylenol or Motrin.
Runny nose and cough- A child who is feeling run down, not sleeping well due to a cold, or actively sneezing and coughing struggles to pay attention and learn.
Skin rash- Take your child to a doctor for a diagnosis and treatment. Treatment must be given for 24 hours before returning to school.
Pink eye/ Discharge from Eyes- If the white of your child's eye is red or has any drainage, you should keep them home and contact the child's doctor. Your student must stay home until they have been on an antibiotic treatment for 24 hours if prescribed.
Head Lice-Children who are found to have head lice at home must be treated before coming to school. Please contact the school nurse for more directions if needed.
Illinois State Board of Education Health Requirements
A physical exam completed no more than one year before the start of the school year is required for all students entering preschool, kindergarten, 6th grade, 9th grade, or entering an Illinois school for the first time. These must be completed and turned in by October 15th.
Updated immunizations are required for students entering preschool, kindergarten, 6th grade, and 12th grade. These must be completed and turned in by October 15th.
A dental examination is required for students entering kindergarten, 2nd grade, 6th grade, and 9th grade. Dental exams must be completed within the 18 months prior to the May 15th due date.
A vision exam is required for all students entering kindergarten or entering an Illinois school for the first time. The exam must be completed no more than one year prior to the start of the school year. The exam must be completed and turned in by October 15th.
The Illinois State Board of Education will allow for immunization exemptions for medical and religious reasons. We would be happy to help you obtain the required physician’s documentation. Please call the nurse’s office at The Center for assistance.
