Health Insurance

Flexible Spending Accounts

  • Allows you to set aside a portion of your pay, before taxes, to cover eligible out-of-pocket health care and/or dependent care expenses


403(b) and 457 SMART Plans

  • Contributions to the 403(b) and/or 457 SMART retirement plans are optional and in addition to the mandatory contributions made to the MA Teachers Retirement System (MTRS) or Beverly Retirement Board


Long Term Disability

  • Replaces a portion of your income if you’re unable to work because of a covered disability

  • Benefits begin as soon as 180 days from the date of your disability


Accident Coverage

  • Provides cash benefits directly to policyholder for accidents or injuries; family coverage is available

  • Coverage includes payouts for ER visits, x-ray, ambulance, fractures, stitches, concussion, dislocations, burns, physical therapy, accidental death and more


Critical Illness Insurance

  • Provides lump sum benefit in the event of a critical illness; pre-existing conditions apply

  • Covered conditions include heart attack, stroke, cancer, renal failure, ALS, Alzheimer’s, comas and more

  • Wellness benefit of $50 per year for cancer screening testing


Group Life and AD&D Insurance

  • $5,000 in life insurance coverage for employee; $5,000 employer match while actively employed

  • AD&D insurance coverage amount matches life insurance amount

  • Voluntary additional coverage available


Whole Life Insurance 

  • Individual Whole Life Insurance Policy with level cost and coverage; spouse, child and grandchild coverage available

  • Plans are portable and can go with you if you leave your job; provides lifetime coverage that builds cash value

Health, Dental and Vision Insurance

  • Enrollment forms must be submitted online within 30 days of your date of hire.

  • Health insurance is effective on your date of hire.

  • Dental and vision insurance are effective on the first of the month following your date of hire.


Flexible Spending Accounts

  • Enrollment form must be submitted online within 30 days of your date of hire.

  • Mid-year enrollments for new hires are for the remainder of the current calendar year.  For example, if you are hired in September and enroll as a new hire, indicate the total amount you'd like to contribute through December 31 of the current year.


Additional Plans

  • Enrollments must be submitted online within 30 days of your date of hire; forms are submitted directly to Membership Benefits Group (details noted on tab below).


Choosing Not to Enroll

  • If you choose not to enroll in benefits at the time of your hire, you will be able to do so during the next applicable open enrollment period.

  • Additional plans such as Critical Illness, Life and Short/Long Term Disability insurance may require Evidence of Insurability (EOI) documentation during later enrollment periods.

Flexible Spending Accounts

  • Open enrollment takes place each November with any changes, non-renewals or new enrollments taking effect January 1.

  • If you are currently enrolled in a Health and/or Dependent Care account, re-enrollment for the following year is not automatic. You'll need to re-enroll by accessing your CPA account online in November.


Health, Dental, Vision and Additional Plans

  • Benefits open enrollment takes place each May with any changes, cancellations or new enrollments taking effect July 1.

  • This is the only time of year for eligible employees to enroll themselves, a spouse or dependents without a qualifying event.  Take a moment to review your current benefits and make any necessary changes, cancellations or new enrollments.  If you’re happy with your current benefits, no action is needed.  Your coverage will continue as usual.  


403(b) Plan and 457 Smart Plan

  • New enrollments and plan/contribution changes: allowed at any time; effective the next payroll

  • Stopping contributions: allowed at any time; effective the next payroll

Employee enrolling in and/or adding dependent to insurance due to qualifying event

  • Enrollments outside of the annual open enrollment period are only permitted due to qualifying events:

    • Loss of previous coverage

    • Change in household: marriage, divorce, birth/placement, death in family

    • Employment status change to more than 20 hours per week

  • Must be completed within 30 days of qualifying event

  • Review benefit summaries and costs

  • Select plan(s) you wish to enroll in and complete the applicable enrollment form(s).  In order to submit the enrollment form(s), you must attach a legible copy of proof of loss of previous coverage, as well as birth certificates for each person covered as well as a marriage certificate, if applicable.


Employee turning 26 and enrolling in our insurance

  • Must be completed within 30 days of loss of previous coverage

  • Review benefit summaries and costs

  • Select plan(s) you wish to enroll in and complete the applicable enrollment form(s).  In order to submit the enrollment form(s), you must attach legible copies of both your birth certificate and proof of loss of previous coverage.


Employee's dependent turning 26 and losing coverage on our insurance

  • You will receive a letter with information and cost for your dependent continuing on our health and/or dental insurance through COBRA

  • Your dependent can use this letter as proof of loss of coverage to enroll in their employer's insurance, if applicable

Benefits can be cancelled on the last day of any month by completing this Benefits Cancellation Form.

Overview

  • Flexible Spending Account plans are administered by Cafeteria Plan Advisors (CPA).

  • Enrollment for the following calendar year takes place each November.

  • New hires are able to enroll within 30 days of their date of hire.

  • If you are currently enrolled in a Health and/or Dependent Care account, re-enrollment for the following year is not automatic.  You'll need to re-enroll on the CPA participant portal each November.


Health Care FSA

  • Eligible expenses

  • OTC expenses now eligible

  • Benefit cards are issued by CPA that can be used at most medical facilities, dental offices, optical shops and pharmacies to pay for eligible expenses. The cards will reload each plan year that you re-enroll.

  • Alternatively, claim forms can be completed and submitted to CPA for eligible health care expenses.


Dependent Care FSA

  • Claim forms are completed and submitted to CPA for eligible dependent care expenses.

403(b) and 457 smart plans


403(b) Plan

We offer a 403(b) retirement plan to employees regularly scheduled to work 20 or more hours per week.  

  • Participation in the 403(b) plan is completely voluntary and separate from mandatory retirement contributions.

  • New enrollments and plan/contribution changes: allowed at any time; effective the next payroll

  • Stopping contributions: allowed at any time; effective the next payroll


OMNI is our third party remitter, and detailed information on our 403(b) plan and participating investment providers can be found here.  

Prior to completing a Salary Reduction Agreement with OMNI, you will need to establish an account with at least one of the listed investment providers.  You are able to contribute to multiple accounts through OMNI.

Before getting started, we recommend you review this FAQ page.  Helpful forms that can be found on OMNI's website include:


  • Salary Reduction Agreement: start, change or cancel contributions

  • Exchange: movement of funds from one 403(b) provider to another eligible 403(b) provider under our plan

  • Transfer: movement of funds from previous employer's 403(b) plan to our 403(b) plan 

  • Rollover: movement of funds to a 403(b) account from any other type of account such as an IRA, 401(k), 401(a), etc.

  • Distribution: cash or rollover disbursement from our plan with a qualifying event (either 59 1/2 years of age or separation from service)


457 SMART Plan

The City of Beverly offers a 457 SMART plan to employees regularly scheduled to work 20 or more hours per week.  Participation in the 457 SMART plan is completely voluntary and separate from the contributions to the plans noted above.  Detailed plan information can be found here.
