You do NOT need to be CNA certified. You will need to have a valid driver’s license and proof of car insurance and need to be at least 16. 

 

GENERAL DUTIES:

·         Acts as a positive role model for the Members they serve. A positive role model demonstrates agency philosophy in regards to positive treatment of Members and teaching of self-advocacy skills.

·         Members are treated with dignity and respect at all times.

·         Communicate in a positive way; and approach all situation with a non-judgmental attitude, such as not imposing personal values on the Members

·         Adheres to designated work schedule, e.g. attends work on time, completes shift as scheduled, alerts supervisor to all schedule changes before change occurs.

·         Maintains scheduled assignments with minimal change of venue requests by the Member, and/or family.

·         Adheres to agency Absentee/ Tardiness Policy. 

·         Adheres to Agency Call In Procedure.

·         Completes all designated trainings within required time frame, keeping all certifications current.

·         Provides services as designated in NCI Lifestyle Plan. 

·         Adheres to specified means of documentation and completes all aspects of programming.

·         Displays initiation while working with a Member whether in the home or the community.

·         Adheres to designated activity schedules as determined by the Member.

·         Communicates pertinent information to the Supervisor in a timely manner (i.e. Concerns, Accident/ Incident Reports, Program suggestions, etc.)

·         Completes all Waiver documentation accurately and in a timely manner (within 24 hours from the end of the shift).

·         Maintains Member and family confidentiality. Protected information (i.e. A Member and/or the family’s conversations with staff, their medical or social histories, any identifying information, treatment programming, etc.) must be treated with dignity and respect, and not communicated to unauthorized persons without the consent of the Member and/or responsible party.

·         Maintains a professional relationship with Members, their family, and others in the community.

·         Communicates in a professional manner with Co-workers, Supervisors, and Coordinators.

·         Implement only designated Member restrictions (if any). Staff will alert Supervisor when a Member and/or guardian, decides to disregard a restriction.

·         Follows the agency chain of command when concerns or problems arise.

·         Acts as a Mandatory Reporter.

·         Understand, educate, and honor Member rights.

 

HOURS:

·         Flexible hours, must be able to work evenings, weekends, and holidays.

·         Staff is scheduled in Members’ homes on an as needed basis
