Dear Parent / Guardian of 5th Grade Student:
Please be advised that in accordance with N.J.S.A. 18A:40-21.2, the New Jersey Department of Health and Senior Services requires the following two vaccines for all students entering sixth grade:
Meningococcal vaccine, one dose
Tdap vaccine (booster), one dose
Students will not be allowed to enter sixth grade without a Health Care Provider’s documented proof of these vaccines. Please send in a copy of the immunization record immediately after these immunizations have been administered. This documentation should be given to your child’s present school nurse no later than June 1st of the current school year.
Additional Note: Many physicians will not administer these vaccines unless the child is 11 years of age. For June, July or August birthdays please provide the school nurse of the appointment date with your pediatrician. Once completed, you would then need to drop off a copy of the immunization record directly to the WAMS nurses.
