Q: How do I make a Bias Incident Report?
A: There are QR codes posted at every BLS school site. Look for them at the main office (if your school site does not have QR posted, let us know!), scan it and follow the steps. There is also a BLS created app found on every iPad where you can report incidents.

A: Adults, family members, staff and even students are welcomed and encouraged to submit reports citing instances of bias at Bend-La Pine Schools.

A: Yes, if the person who is submitting the Bias Incident Report wishes to remain anonymous they can. However, if any follow-up work is requested it won't be able to be fulfilled due to the nature of the anonymous report.
