There are different types of seizure disorders and we want to make sure we are prepared to help you or your child in the best and safest way possible. Providing the school with a Seizure Action Plan will help us understand the type of seizure disorder your child has and determine the best way to manage their seizure when one should occur. If you plan to keep emergency medication at the school, a medication permission request form should be completed as well. Please complete the forms below and return to the school nurse.
