Any student with diabetes must have a diabetes management and treatment plan (DMTP) on file for each school year. This plan should be developed by the the student' s parent or guardian AND the physician responsible for the student's diabetes treatment. The forms below should accompany the DMTP and be turned in to the school nurse.
