Texas law allows students to carry and self administer asthma or anaphylaxis prescription medications if the physician has determined the student has the necessary skill level to self-administer. Written authorization signed by the parent or guardian AND the physician is required. Please complete the forms below and return to the school nurse. At this time we do recommend the use of spacers for those that carry inhalers.
