Student Medication Information for Washington D.C. Trip , May 2019
- Students may carry over-the-counter medication in small amounts such as Tylenol, Advil, and allergy medications
- Prescription Medications require a signed parent permission form (Form C)
- Prescription Medications require a written doctor’s signature (Form D)
- Prescription medications will be held and administered by the field trip nurse.
- Please get prescription medications and the signed paperwork to the JGMS nurse’s office by Monday May 13th.
- All medications must be in the original labeled medication/prescription bottle (this includes over-the-counter medication) We don’t accept medication in baggies or unlabeled containers.
- Pack only enough medication for the trip.
EMERGENCY MEDICATIONS (EpiPens/Inhalers)
- For Epipens- we will send a copy Emergency Action Plan that is on file at the nurse’s office with the field trip nurse (you do not need to send in forms C & D)
- Inhalers or EpiPens should be held by the student.
- Please remind students to pack it with them each day (Don’t leave in hotel room).
