Good Faith Estimate
Under Section 2799B-6 of the Public Health Service Act, you are entitled to receive a “Good Faith Estimate” of expected charges for services. This applies to those who are not using insurance or are not enrolled in a plan, and this information can be provided upon request or at the time of scheduling.
Your Right to a Good Faith Estimate
By law, health care providers are required to give an estimate of the total cost of medical services to patients who are not using insurance.
You have the right to receive a written Good Faith Estimate for any non-emergency service, which includes related costs such as medical tests, prescriptions, and equipment.
Your provider will give you this estimate at least 1 business day before your appointment.
You may request a Good Faith Estimate before scheduling services.
If your final bill is $400 or more above the estimate, you have the option to dispute the bill.
Please save a copy of your Good Faith Estimate for your records.
