BSD & Club Requirements:
1- Athlete's Participation Application: Signed by Parent/Guardian
2- *Medical Release: Physical Exam (Good for 2 years)
3- Transportation Release Forms
Personal Vehicle Release
District Vehicle Release
4 - Activity Fee (Optional)
***FORMS CAN BE PICKED UP IN THE ATHLETICS OFFICE and/or SEE COACH GRAY***
