What needs to be done by Aug 1st to participate?

1.  Final Forms need to be completed by parent and athlete https://beavercreek-oh.finalforms.com 

2.  Must have a current physical on file

3. All athletes, grades 7-12 and high school band/color guard/percussion ensemble (band-related) members are required  to have a valid OHSAA physical on file in FinalForms in order to participate in Beavercreek athletics and/or band.

4. Athletes and band-related members may not participate in practices, scrimmages or games until all forms are completed, electronically signed and submitted.

5. Physical form is valid for 13 months after the date of physical exam.

6. Physical form is valid for each sport until it expires; no need to submit a new physical for each sport.

Team Fee & Pay-to-Play Fee

1. Team Fee is $50 for grades 9-11 and $65 for seniors . That covers the cost of warm-up top, Practice tank top, money for Coach's gift at the end of the year and a few other cost. FEE IS DUE BY AUG 9TH. ATHLETES THAT DON'T PAY WILL NOT GET THEIR UNIFORM TOP AND CANNOT PARTICIPATE IN TEAM DANCE PARTY ON THE 11TH. CHECKS MADE TO BCCPA.
