Athletic Forms

Please ensure the NCHSAA physical form (pages 1-4) is completed and uploaded into your students Dragonfly account.

• Pages 1 and 2 - Health History Form (completed by a parent/guardian) *can be completed within Dragonfly

• Page 3 - Physical Examination Form (completed by a health care professional) *upload in Dragonfly

• Page 4- Medical Eligibility Form (completed by a health care professional) *upload in Dragonfly

The following forms should also be completed in Dragonfly as well:

• NCHSAA Gfeller-Waller Student-Athlete & Parent/Legal Custodian Concussion Information Sheet *can be completed within Dragonfly

• NCHSAA Eligibility, Consent to Participate and Release Form *can be completed within Dragonfly

• Brunswick County Schools Athlete Registration Form *can be completed within Dragonfly
