Please complete the front of the school health physical form and take to the doctor for the physical exam portion to be completed.

Please complete this survey using the Code: abc123 for all Pre-K students.

This form needs to be completed for all students registering for Pre-K and residency documentation attached to the form.

This form needs to be completed by all students registering for Pre-K if ultilities are not in the name of the parent/guardian. This form needs to be notarized at the Central Office.
