a Senior Professional Educator License

The Ohio Dept of Education requires a verification form. You will need to email Kristen Huston requesting the verification form with your "official name" for your license and your Ohio Educator State ID (License Number).   Please note in the sheet below, I hid the 1st 5 years in order to see the end date for all.
