*PLEASE NOTE:  Parent/Guardian signature is required for BOTH forms if the volunteer is a minor (under the age of 18).

• Every volunteer, chaperone, contractor working with students, and every employee is required to complete a background check and sign a Child and Youth Abuse Prevention (CYAP) Policy acknowledgment.
  
  
  

• Background checks must be completed to ensure the organization has updated information on the individuals serving in our church and school. These records are processed and stored securely by the Human Resources Department of Bay Area Church & Christian School.
  
  
  

• Please note that a background check can take up to one week to be processed. Once a campus volunteer coordinator has contacted you regarding this volunteer screening, we kindly request that you complete the two required forms (linked above) as soon as possible to allow adequate time for processing.
