*PLEASE NOTE:  Parent/Guardian signature is required on the form if the volunteer is a minor (under the age of 18).

• Every volunteer, chaperone, and contractor that works with students, along with every employee, is required to complete the appropriate document(s) as indicated above.
  
  
  

• Background checks must be completed to ensure the organization has updated information on the individuals serving in our church and school. These records are processed and stored securely by the Human Resources Department of Bay Area Church & Christian School.
  
  
  

• Please note that a background check typically takes up to one week to be processed. Once a campus volunteer coordinator has contacted you regarding this volunteer screening, we kindly request that you complete the required document(s) linked above as soon as possible to allow adequate time for processing.
