High School Student Enrollment Forms
This page contains all forms found in the High School student enrollment process.
9th - 12th Grade
NEW & EXISTING STUDENTS
All forms below labeled REQUIRED must be completed for NEW and EXISTING students.
NEW STUDENTS ONLY
For NEW students ONLY, the forms below must also be completed (in addition to the forms above).
For each NEW student enrolled, the following documents must be submitted:
- Shot Records
- Birth Certificate
- Social Security Card
- Two Proofs of Residency (utility bills) (phone bills not accepted)
You may submit the documents using any of these options:
- Email documents to watsonc@bauxiteminers.org
- Upload documents with this form (requires a Google/gmail account)
- Mail documents to 800 School Street, Bauxite, AR 72011
- Fax documents to 501-557-2774
- Drop off documents at BHS Office during school hours
