Does the school nurse already have the necessary forms for my child's daily or emergency medications?
Yes: You are all set! No new paperwork is required for the trip.
No: You will need to use the BASD Medication Authorization Form for any medication your child will need to take while on the trip IF they do not currently take it at school.
Do you want your child to have access to the over-the-counter medications that will be available on the trip?
Yes: Please complete the senior trip over-the-counter medication form and check the medications you would like to be available.
No: Please complete the senior trip over-the-counter medication form and indicate that you do not want your child to have any of the listed medications.

School district policies, PA DOH, and PA DOE guidelines and rules that apply in school also apply to any school trips. Thus, students are not permitted to carry medication, other than emergency medications (epinephrine autoinjectors and inhalers) and diabetic supplies.
If your student needs to take any medication, including vitamin supplements or other over-the-counter medication (e.g., allergy medication, travel sickness medication, e.g., Dramamine), you will need to supply to the school at least one week BEFORE departure the following:
A correct, complete healthcare provider's order for the medication
The medication in the original pharmacy-labelled container or, for over-the-counter medicines, the original packaging
A medication order needs to include the following information:
Dates of order: ONLY for the duration of the trip (i.e., 4/23/2026 to 4/26/2026)
Student name
Medication name
Strength
Dosage (for over-the-counter medication, dosage must be specified; the order cannot state "follow directions on box/packet/label")
Route (e.g., oral, nasal, topical)
Timing/Frequency
Reason for taking the medication
Possible side effects

A Licensed Nurse will be going on the trip and administering medication. She will not be able to administer any medication that does not have a correct provider order.
