Every Senior's Parent/Guardian must fill out this form. This form is due on Tuesday, April 28th. 

All community service forms must be turned into Guidance no later than 3:15 pm on Friday, May 1st. 

If you have a conflict on any of the Senior Final Exam days, you must complete this form to reschedule that final with your teacher. This form is due by 3:15 pm on Friday, May 1st 

If you are bringing a guest who is not a Bellefonte student, you must complete the Outside Guest Form and turn it into the student office by May 8th at 3:15. 

Students can print and bring this form with them on Thursday, May 21st. Students need to know how many handicap seats and/or handicap parking passes they will need for guests for the Awards & Graduation Ceremonies. Please arrive at school according to the time listed for your last name. 

If you are interested in purchasing your Chromebook for $25, you must bring this SIGNED form to "Senior Check-Out" Day, along with your Chromebook and $25 (cash or check made out to BASD. If you do not want to purchase your Chromebook, bring your Chromebook and charger to "Senior Check-Out."
