All Tri-M members must fill out a service log form to receive their hours for volunteer events. Please fill out this form once you have finished your event shift and are ready to leave. Members are required to take a BEFORE AND AFTER picture with another Tri-M member (unless there is only one spot for the shift). 

Filling out this form is REQUIRED in order for you to receive your service hours! If you miss your event submission deadline, your hours will not be recorded.
