Community Service Hours Verification Forms

You must record all your hours both on paper and online in order to receive credit. You must also get a signature from the organization which you volunteer for in order to verify your hours served. You may submit your hours to your history teach each time you complete an activity or you may choose to record all of our activities on one sheet and turn them in at the end of the year.
