Residency Forma A & B

If you reside with someone else, you will be required to complete the Proof of Augusta County Residency (Notarized Form A and B) and submit a copy of the householder’s lease agreement and/or mortgage contract.

Informal Caregiver Agreement

If you are temporarily caring for a child without an existing Legal Document or Court Order granting Custody of the child, please complete this form, to be signed with the current legal guardian before a Notary.
