Residency Forms A & B

If you reside with someone else, you will be required to complete the Proof of Augusta County Residency (Notarized Form A and B) and submit a copy of the householder’s lease agreement and/or mortgage contract. 

*Please note: Both required forms MUST be signed in person before a Notary in order to be accepted.

Informal Caregiver Agreement

If you are temporarily caring for a child without an existing Legal Document or Court Order granting Custody of the child, please complete this form, to be signed by both the caregiver and the current legal guardian, in person before a Notary, in order to be accepted.
