Understanding self-advocacy is a key step in gaining the confidence and tools needed to navigate challenges associated with living with a disability. Through self-advocacy, individuals can take charge of their education, career, and personal goals—empowering them to speak up for their needs, make informed decisions, and ensure their voices are heard.
Our self-advocacy services are available to individuals ages 14-23, and we offer support tailored to your specific needs.
For Students (Ages 14-23)
If you are a student between the ages of 14 and 23, have been diagnosed with a disability, and are still attending school, you are eligible to receive self-advocacy services. This service is available through ACCES-VR (Adult Career and Continuing Education Services - Vocational Rehabilitation).
Steps to Get Started:
Complete the Referral Form:
For individuals 18 and older, please complete the referral form in MS Word or PDF format.
If you are under 18, a parent or guardian must complete the form on your behalf.
Download the Form:
You can download the form and open it with Adobe Reader or MS Word.
Fill Out the Form:
Ensure all required information is completed.
Sign the Form:
Print the form and sign where indicated.
Return the Completed Form:
Once completed, return the form to the appropriate agency for processing.
If you are not currently a student but would still like to receive self-advocacy services, you can apply through ACCES-VR.
