If you want to loan any equipment listed above, please complete the following form. We will provide the devices on a first-come-first-serve basis.
AACcess Loaning Requirements
AACcess Loaning Requirements
Due to the high price of these items and to meet the grant guidelines, we have enacted the following requirements:
Devices may only be loaned to professionals and providers who have signed a short-term or Agency Agreement.
Devices and accessories must be used carefully and responsibly and protected from damage or loss while in the Agency’s possession.
The Agency assumes responsibility and is liable for replacing any item(s) lost, stolen, or broken beyond repair through misuse while in the Agency’s possession.
Devices and accessories may not be loaned, sold, disposed of, or provided to persons other than the Agency’s authorized user(s).
The loan length is two to four weeks, and it is the responsibility of the Agency to return to ATLA by the due date to avoid late fees.
The borrower must complete a Data Gathering Form at the end of their loan period before their Agency can begin another loan.
The borrower can only loan one device purchased through the AACcess grant.
This project was funded by a grant from the Alaska Mental Health Trust Authority. Alaska Mental Health Trust Authority is a state corporation that administers the Alaska Mental Health Trust, a perpetual trust, to improve the lives of beneficiaries.
