Only persons of legal age are allowed to request for Academic Documents.
(a) If you are the parent/guardian making the request, please provide copies of valid government IDs of parent/guardian and student.
(b) If you are the student (of legal age), please provide a photocopy of a valid government ID with a visible date of birth.
(c) If you are the student (minor), please provide a letter of authorization from your parents/guardian authorizing the request, a valid government ID of the parent/guardian, and a valid government ID of the student.
(2) Accomplish the Academic Document Request Form. Please include your FULL NAME, ID Number (if known), academic years covered in the request, number of requested copies, and document format (PDF or hard copy). Corresponding fees are indicated in the request form. Your records will be checked. Please allow 7-10 working days for our office to accomplish your request. Incomplete records may take longer retrieval time. Payment instructions will be sent.
(3) Pay for the fees. Online payment is the only option allowed by the University. Send the proof of payment via email. The Accounting office requires 3 working days for the payment to be verified. Once verified, the Records Office will get in touch with you for academic document retrieval.
(4) Document Retrieval Options (Please coordinate with our office; campus access is limited up to this time.)
In-person with Campus Access (Records Office - 8:00am - 11:30am; 1:30pm - 4:00pm, except weekends and Holidays)
In-person without Campus Access (we will apply for a Campus Access on your behalf. Campus Access Approval Required)
Gate 3 pickup (Wednesdays 2:00pm - 3:00pm only)
Courier Pick-up (Courier Fees/Payment c/o Requesting Party - Please coordinate with our office)
(Authorized Representative pickup - please provide an authorization letter, copies of valid IDs of document owner, requesting person, and authorized representative)
NOTE: Unclaimed records requested from the Office of Registrar will be disposed of after 60 calendar days (2 months). Please accomplish another Academic Document request for future transactions.
