The cost for an initial 90 minute session is $150. Individual sessions are $120.
Payment is expected at the time of visit. Accepted forms of payment include cash, check and credit card. If you would like to utilize insurance, we can assist you in filling out the necessary forms for you to submit to insurance.
All cancellations must be made with 24 hours prior to appointment time. If the appointment is not cancelled with 24 hours notice, or if there is a no-show for the appointment, a $75 fee is charged. You will be required to have a card on file to take care of these charges if this occurs.

Please print and fill out all forms prior to your first office visit. If you are unable to do this, please arrive 30 minutes prior to session to fill out paperwork. Filling out this paperwork prior to arrival ensures that your session is spent focusing on clinical issues.
