Create a New Care ProvideR

You can create a new Care Provider using the "+ New Care Provider" option located under the "Providers" tab. 

Begin entering the basic information on your care provider at the top left of the page. This will be the contact information for the actual provider (physical office location).

Be sure to make this provider Active using the Provider Status on the right side of of the page and select a Provider type*.

After completing the basic information scroll down to the Company Information you can do 1 or 2 things when entering this information. 

#1: If the company information differs from the contact information enter in the specify compay information. I.e the company is owned by another entity name (Brookstone Hospital is Care Provider Name and owned by Mayo Clinic the Company Name will be Mayo Clinic).

After completing the basic information scroll down to the Company Information you can do 1 or 2 things when entering this information. 

1. If the company information differs from the contact information enter in the specify compay information. I.e the company is owned by another entity name (Brookstone Hospital is Care Provider Name and owned by Mayo Clinic the Company Name will be Mayo Clinic). 

2. If the Care Proivder name and Company Name are the same you can select the Provider button to copy the information from the top basic information section. This will auto populate the Company information portion.

Next fill out the Contact Information. This will be the contact information for the actual location you are entering. You ca enter in your point of contact in this section or put the receptionist information here. You also have the option to copy information from the Company or the Provider to auto populate the fields.

After filling out the contact information select 2 General & Medicaid and  3 Final Information to fill out the remainder of the care providers profile then hit SAVE in the top right corner.
