Who needs an emergency Action plan?
Who needs an emergency Action plan?
If your student lives with a chronic health condition, such as Asthma, Allergies, Seizures, Diabetes, or any other health condition, they may need an individual health plan and/or and emergency action plan. Please discuss with your student's doctor and provide your student's school with an individual health plan if needed along with any emergency medications that your student needs .
The form must be completed on a yearly basis for each student. Please make sure to provide the clinic with an up to date ARGS Student Health Form by the first day of school.

No medications will be provided by ARGS. Parents must provide any medication that their student needs while in school.
