What do I do with my completed physical forms?
You have three options:
1- Have your athlete bring it to his/her first practice/tryout
2- Upload it to Final Forms electronically (Still turn in a hard copy to your son/daughter's coach)
3- Turn it in to the high school athletic office or middle school main office
Registration for the 2026-2025 sport seasons are now open on Final Forms for incoming 7th-12th graders and will be open for the general student body on June 30, 2025.
Parents and students need to review their forms online and provide electronic signatures at the bottom of each form to verify information is current. Print the athletic physical form and take that to your doctor. The physical packet is the only form that needs to be printed and returned to the athletic office. You can add new information or update information at any time during the year.
For incoming students in grades 7-12 that plan to play sports, the parent and student registration needs to be completed as soon as possible in order to begin fall practices.

Please schedule your athletic physicals with your family doctors as soon as you can, before Aug. 1 and turn in to the athletic office either at the high school or middle school.
Physical forms need to be printed off Final Forms (once you update the information), taken to doctors and returned to the athletic office.
