In Step 2 you will complete a Google Form in order to complete the EMS course request process.
A parent/guardian AND the students should complete this form together. You can only submit this course selections Google Form one time. Please allocate 15-20 minutes to complete this form as it needs to be completed in one sitting. Your work will not save if you start and come back to complete later in the day.

Preview electives offerings on the Courses&Programs page; be prepared to request/rank elective choices.
Know your APS student ID number.
How to Fill Out Your Online Course Request
Select the 2023-2024 Google Form link below for the grade-level you will enter in the fall of 2023-2024.
Complete the Google Form; a parent/guardian and student should complete the form together.
This Google Form may take 15-30 minutes to complete; please allow yourself enough time to complete in one sitting to avoid having to restart from the beginning later.

Current EMS Students & Feeder Elementary Schools:
This registration form is only for incoming 6th, 7th, and 8th grade EMS students. If you are selecting courses for more than one child from the same household, you must complete the course selection form separately for each student.

Next Steps
After submitting the Course Selection Google Form, a copy of your response will be sent to your APS student Gmail account.
Please forward this to any parent/guardian email accounts as needed for your own personal records.
