Please remember any suicidal thoughts or ideation needs to be reported immediately and in-person to the school counselor, school nurse, school social worker. If none of those people is available then contact administration, do NOT use this form.

The Health and Wellness team is comprised of the School Counselor, Nurse, Behavior Redirector, and Admin. We work with staff and families to connect them with resources and work as a team to help students succeed. Students can be referred for health (mental and physical wellbeing), behavioral, and/or academic concerns.

Please note In order to hold an Attendance Team meeting a student must have an absence rate of 10% or more AND the referring teacher must have input data, with current interventions (when the student had an absence rate of 5%-9%), into Synergy MLSS as an Attendance Intervention Plan.
