School Immunization Requirements

The Matanuska Susitna Borough School district is bound by Alaska Statute (AS 14.30.125) and Alaska Administrative code (4 AAC 06.055) to comply with the State of Alaska School entry immunization Requirements. By law, a student is not allowed to attend classes unless they are compliant with immunization requirements. The exceptions to this are a valid Alaska state medical or religious exemption (must be notarized) or a 30 day waiver for those who are in the Families in Transition or active duty military dependents who have just been relocated to Alaska. (Military and Homeless waivers required by federal law).

How do I get my immunization records from Alaska? 

If you need a copy of your vaccination record, options include: 

1. Download an electronic record from the Docket app. 

2. Request a paper copy by faxing or mailing an Immunization Record Request Form to the Alaska Immunization Program.

3.  Request immunization record through your healthcare provider.
