Notes from Nurse Candi

·  All students should return a completed Student Health Information form each year. 

·  Medication forms or health care plans should be completed as soon as possible, so they will be on file with me the first day of school. Medication Authorization Forms need to be renewed each school year. (Blank documents may be accessed via the CCS website under School Health.)

·  Immunization records for ALL students should be up to date and on file for school attendance.

·  NC Health Assessments are required to be on file for any student who has never attended public school in North Carolina.

 *** SENIORS:  As of August 2020, NC law requires all 12th graders to have a meningitis vaccine booster for school. Parents/ Guardians of seniors, you will need to provide a copy of your child’s up to date vaccine record documenting it has been received by the 30th day of school.
