PARENT/GUARDIAN AGREEMENT FORM TO PUBLISH STUDENT INFORMATION ON THE SCHOOL DEPARTMENT WEB SITE

The Georgetown School Department maintains an official website to provide general information about our school system, as well as information about educational programs, extracurricular activities, school events, and student and staff achievements.

Maine law requires public schools to obtain written approval from parents/guardians prior to publishing personal information about students on the Internet.

This form will authorize Georgetown School Department to publish the following:

• Student information, including pictures, and cited work. Students will be identified by first name and last initial, or by first name only.

• The signed release form shall remain current for as long as the student is enrolled at Georgetown Central School unless permission is revoked in writing by the parent or guardian.

I understand and agree that my child's first name last initial may appear on the Georgetown Central School web site. I further understand and agree that photographs of my child and/or examples of my child's work may be used on the web site.
