REFERRAL TO PUPIL EVALUATION TEAM - Procedure
STEP I Before Written Referral
1. Contact parent about issues
2. Review cumulative file (i.e., grades, test data, etc.}
3. Document in writing alternatives tried and results, especially written documentation (consult special education staff for suggestions}.
STEP II Written Referral
1. Complete fully written referral (special education staff could be of assistance)
2. Submit written referral to building principal
3. Building principal signs if written referral is complete
STEP III Special Education
1. Written referral sent to appropriate special education staff upon signature of the building principal
2. Special education staff will notify the Special Services Director of referral by sending the original of the referral to the Special Services Central Office.
3. Written notice sent to parent of referral and the date of the initial P.E.T. meeting.
4. The initial P.E.T. meeting will be held within fifteen (15) school days from the date of the Principal' s signature on the referral.
STEP IV Initial P.E.T.
1. Decide whether to evaluate and the possible disability for which the evaluation is being conducted
2. If evaluating, decide on assessment tools and obtain written permission from the parent to test
3. If P.E.T. decides not to evaluate, referral ends.
STEP V Evaluation
1. Must use more than one (1) instrument
2. Must have at least two (2) evaluators
3. Must contain a written observation of student in the classroom
STEP VI Second P.E.T. to determine eligibility for Special Education Services
1. Must be held within forty-five (45) school days of obtaining parent permission to evaluate.
2. Written notice of second P.E.T., including a list of tests given to the student sent to the parent.
3. Parents must have an opportunity to review evaluation results prior to the P.E .T. (i.e., conference with parents just prior to the meeting}
4. P.E.T. determines the student eligibility based on test results and information presented at the P.E.T. meeting. (i.e., does the student qualify under the disability criteria)
5. If student does not qualify, the P.E.T. determination to include statements to that effect plus any recommendations outside special education.
6. If student qualifies, an I.E.P. is written. (The I.E.P. must be implemented within thirty (30) days of the student's identification.)
7. If student qualifies under a learning disability, an L.D. document must be completed.
8. Consent for placement must be signed by the parent/guardian.
Copies of the P.E.T. Minutes and the I.E.P. (including long and short-term goals) must be sent to the parents/guardians within twenty-one (21) days following the meeting.
