Please read the following:

Do you have a Kindergartner or 6th grade student starting school this year?? If so, you need to have your school physical and required immunizations done by the first day of school!!

Kindergarten requirements: An up to date school physical on the Illinois school physical form (with a date in the corner of 11/15), 2 MMRs, 2 Varicella (chicken pox) vaccines, AT LEAST 3 doses of DTaP and Polio vaccine. They will also need a lead screening form. By October 15th all kindergarteners are required to have an eye exam done by a licensed eye doctor. By May 15th of 2021 they will need a dental exam completed. If your child sees Dental Safari that exam will satisfy the dental requirements!

2nd Grade Students: Dental exam by May 15, 2021.

6th grade students: An up to date school physical on the Illinois school physical form (there will be a date in the corner of 11/15), 3 Hepatitis B vaccines, 2 MMRs, 2 Varicella (chicken pox) vaccines, a Tdap vaccine and a meningococcal (meningitis) vaccine. Also every 6th grade student is required to have a dental exam by May 15, 2021. If your child sees Dental Safari that exam will satisfy the dental requirements!

Please keep in mind that:

I will not be able to accept physicals on old Illinois physical forms.

The top parent portion of the physical form must be completed. This has to be done in order for the physical to be considered complete.

ALL sections of the physical form must be completed to be considered complete.

If you haven't scheduled your physical I recommend that you do that today!! Appointments fill up very quickly!!


Medication at School

Some students require medication to be taken at school. If your student needs to have medication given at school, please contact Ms Candice. You and your child's healthcare provider will need to complete paperwork. No medications (even over the counter medications (Tylenol, Ibuprofen, eye drops, cough drops, cough syrups) should be brought to school without a doctor's order. Student's should never self carry any medications.

Inhalers at School

Inhalers may be brought to school and be carried with a student. The nurse must have the following:

  • An order from the student's healthcare provider approving the medication to be at school and carried by the student.

  • An authorization from the student's parent/legal guardian approving the student to self-carry the inhaler.

  • An asthma action plan from the student's healthcare provider.

  • The inhaler in the original box with a pharmacy label attached to it.

Nebulizer treatments may NOT be given at school this year due to aerosolizing. If your student requires a nebulizer treatment, a parent will be called to pick up the student.


Health Exams

All students entering Pre-K for the first time, Kindergarten and 6th grade students must present an Illinois physical dated within the last year to enter school. Also, student entering and Illinois school for the first time must present an up to date school physical on an Illinois form.
